Prescription Refills

Monthly medication refill requests should be made during regular business hours.  Please allow 48 hours for request to be completed and plan accordingly.

Prescription refill requests for non-controlled medications can be made directly to the pharmacy.  The pharmacy will contact us electronically for refill.

Online Screening Tools / CHADIS

We encourage all our parents to register at CHADIS, a confidential online system that allows developmental and health questionnaires to be completed prior to your child’s visit.

These questionnaires will provide your pediatrician with information about your child’s health and development, as well as any concerns that you may have, which will allow a more thorough and efficient evaluation during your visit.
